2008 Membership ApplicationsThe 2008 Membership Applications can now be downloaded from the new website.
Please make sure you fill it out completely and legible. If you are under 18 years of age you must have a parent or legal guardian fill out the bottom and have it notarized. Also include a copy of your birth certificate.
Memberships will also be available at the Maple Shade / D4 Banquet on Jan. 10th in Jamestown.
The memberships are still $20 and the checks can be made out to District 4 MX/ATV and sent to:
